Homeless Management Information System (HMIS) Privacy Statement


YOU HAVE THE RIGHT TO REFUSE TO ANSWER QUESTIONS COLLECTED FOR THE PURPOSE OF HMIS DATA COLLECTION

Agencies participating in the HMIS may ask and require some information from you as a part of regular program eligibility and services they provide. This information may be entered into the ServicePoint database. You are not required to answer any questions asked for the sole purpose of HUD HMIS Data collection and you will not be denied services for refusing to answer the HUD HMIS data collection questions.

WHY WE COLLECT INFORMATION
"We collect personal information directly from you for reasons that are discussed in our Privacy. Notice. We may be required to collect some personal information by law or by organizations that give us money to operate this program. Other personal information that we collect is important to run our programs, to improve services for homeless persons, and to better understand the needs of homeless persons. We only collect information that we consider to be appropriate.
The collection and use of all personal information is guided by strict standards of confidentiality. Our Privacy Notice is posted. A copy of our Privacy Notice is available to all clients upon request."

HMIS PARTNER AGENCY USE OF YOUR INFORMATION
Purpose of Data Collection: Information you provide to this agency will be entered into ServicePoint, the Homeless Management Information System (HMIS), sponsored by the Austin Travis County MHMR Center. You will receive the same services, whether or not you allow your personal information to be shared with other Agencies participating in the HMIS database.

  • Your personal information that is in the HMIS will not be shared with any other people or organizations, even other HMIS Partner Agencies, unless you say it can be.
  • Your personal information that is in HMIS will not be shared with any government agencies without your consent, except as required by law.
  • Information is collected for some of the following reasons: To improve, provide, and coordinate services to individuals, to carry out administrative functions, and to create de-identified (anonymous) information that can be used for statistical purposes without identifying clients; such as assisting the community in counting the numbers of homeless persons served per year.

What information is collected about you?
Depending on your situation, you may be asked for some or all of the following:

  • Basic identifying information including: name, SSN, date of birth, gender, race, marital and family status, household relationships, phone numbers, military veteran status, whether or not you have a disability;
  • Housing information (may include address, type of housing, homeless status, reason for homelessness)
  • Income information (sources and amounts of household income, employment information, work skills)
  • Legal history/information
  • Medical Information
  • Services needed and provided; outcomes of services provided

YOUR RIGHTS & CHOICES
What happens to your information?

  • When you request services from this agency, your information will be entered into the Austin Area ServicePoint system, which operates over the Internet. ServicePoint uses many security protections to ensure confidentiality and only agencies that use ServicePoint in Austin can access this program.
  • Agencies participating in the HMIS may ask and require some information from you as a part of regular program eligibility and services they provide. This information may be entered into the ServicePoint database. You are not required to answer any questions asked for the sole purpose HUD HMIS Data collection and you will not be denied services for refusing to answer the HUD HMIS data collection questions.
  • You have the right to change your mind about what types of personal information about you this agency can share, and who they can share it with. You must notify this agency in writing if you have consented to the sharing of information and have changed your mind.
